Purpose of an Amendment

Amendments are used to make corrections, clarifications, or updates to an existing clinical note.
They ensure documentation remains accurate while preserving the integrity of the original entry.


Key Principles

  • The original note is never deleted.

  • An addendum is added to the original note.

  • Every amendment is dated, time-stamped, and signed, maintaining a clear audit trail.


How to Add an Amendment in PatientStudio

1. From the Patient Profile. Select the applicable case.

2. Click "View Note" for the applicable DOS

3. In the upper righthand corner, Click three dot icon.

4. Click "Add Amendments"

5. Click "Continue"

6. You should see the "You are amending" alert.

7. When finished, click "Submit Amendment"

8. Click "Confirm"
